⭐Just a reminder...make sure to upload all required documents, including the yearly Sports Physical packet, to Aktivate.  Even if your child is only participating in one sport, we recommend selecting all four sports.  If you select only one sport and later decide to participate in additional sports, you will have to register again.  ⭐

To participate in the Athletics Program, students must have an OCPS Sports Physical turned in for the specific school year and have a cumulative MS 2.0 GPA.

Medical eligibility and parental consent

All sports registration sections must be completed and submitted online BEFORE you can participate in any way (including tryouts).

Each student must be registered in the Aktivate system

• Please complete sports registrations/forms online in the Aktivate system.
• Upload a current sports physical into Aktivate.

• These are the ONLY ACCEPTABLE forms from the doctor. Once completed by the doctor, scan or take a picture and upload into  Aktivate. 

The registration process includes sections for parental consent to participate and a section to download/print the physical form to take to the doctor as well.  Please upload the completed form into the student registration.

*NOTE: When you register on Aktivate, select all 4 sports so that you will only have to do one registration. If you only select one sport and decide to participate in additional sports, you will have to register again.

No additional “ECG” or “EKG” exam is needed.
